USCG approved Drug Screen results   

 (valid if issued within 1 year of being issued) or letter from current employer

Copies of:

· License Training Certificates

· TWIC (

· CPR/First Aid course Cert (valid if issued within 1 year of being issued)

· Driver License

· Social Security Card (optional)

· Birth Certificate (optional)

(previous USCG License; if applicable)

-Renewal - $95

For fastest turn around time to the NMC, please include

How to submit for review:

1. Scan ALL completed documents to a .PDF file
